Understanding your health is a commitment of time, energy, and finances, we encourage you to verify if our health benefits are part of your insurance plan so that you understand your coverage at the time of treatment.
Please understand we offer insurance verification as a courtesy, however, sometimes it is not 100%accurate. After billing, if the insurance does not pay, it is the patient's responsibility to pay the services fees. A credit card will be held on file.
Payment, whether it be, cash, check, major credit card, or co-pay is expected at the time of service. Credit cards are subject to a fee. Cancellations or missed appointments without a 24-hour notice are subject to a $30 Cancellation Fee. Unfortunately, Insurance companies do not pay charges for missed appointments. Other payment options may be considered if needed.
The information requested is part of the process of working together and includes a comprehensive look at your health and lifestyle history, all of which contribute to an optimal outcome.
If you are a new patient, please complete and sign all forms and be sure to bring your insurance card and driver's license to your initial visit.
